Services of AARP are insurance and discounts on drugs through the
National Chapter; information and referral sessions on health;
nutrition; fire protection and prevention; social security; and
outings. Classes on 55 Alive/Mature Driving, to help Senior
drivers be safer on the roads and to become better drivers.

AgeQuest, LLC provides assistance to adult children and family
caregivers on New Hampshire's Seacoast who are bumping up against
the challenges of aging. The Seacoast is blessed with excellent
long-term care providers. We help families make the most of the
resources available by providing assistance with needs
assessment, planning, and help with implementation, monitoring
and fine tuning as changes occur. We take a "coach"
approach - tailoring our services to give the level of support
our clients want. We meet in our office at worksites, in private
homes, or extended care facilities.
Nancy Euchner has over 25 years experience in caring for the
elderly in a variety of settings, a passion for working with the
older people and their families, and a real understanding and
respect for the challenges that caregivers face.
We are familiar with the long term care providers of the greater
Seacoast area. Our network includes traditional providers, and
also those with new ideas to provide practical help to elders and
caregivers. If you have a need outside this area, we can use our
contacts to link you up with someone who can help.

AIDS Response-Seacoast (ARS) is a non-profit, community-based
HIV/AIDS Service organization dedicated to providing education,
direct assistance and advocacy for persons and communities
affected by HIV/AIDS. We provide direct service to clients at all
stages of HIV infection who live in Rockingham, Strafford' and
southern York counties and educational programs throughout New
England. ARS evolved as a grassroots response to the epidemic in
1987 to offer vital emotional and practical support to people
living with HIV infection and AIDS.

The American Red Cross provides the following services: Disaster
Relief and Education - this includes responding to disasters and
providing relief of clothes, food, shelter, and mental health
counseling to victims of disaster; Armed Forces Emergency Service
- this program links active military men and women with their
families: Blood Services - the Red Cross is the leading supply of
blood and blood products to our community hospitals; Health and
Safety - this program provides life-saving training such as First
Aid, CPR, swimming, life guarding, and First Responder. The
program also sponsors an LNA (Licensed Nurses Assistant) Training
course as well as a youth tobacco education curriculum. The
lifeblood of the Chapter and its operations in the 39 cities and
town on the Seacoast are the volunteers who join our cause and
help us complete our mission. The Great Bay Chapter serves the
counties of Rockingham and Strafford in New Hampshire and the
towns of Eliot, Kittery, Kittery Point, and Lebanon, Maine.

A community based health center with the mission of providing
quality healthcare for a diverse population, irregardless of
their ability to pay. We provide family and primary healthcare,
prenatal care, child health care and immunizations, reproductive
healthcare, women's healthcare, and supporting social services.
We accept NH Medicaid, Maine Medicaid, Medicare, and most
insurances; and a sliding fee scale for the uninsured. Discount
prescriptions are available based on qualifications. Free Breast
and Cervical Cancer screenings for qualified women. WIC and CSFP
supplemental nutritional programs are available in both Dover and
Rochester. Sexually Transmitted Disease Clinic and HIV/AIDS
Testing and Counseling held weekly in Dover on Tuesdays from 5:00
to 6:00 pm, and in Rochester on Thursdays from 5:00 to 6:00 pm. NEW:
Dental Services now available in addition to our primary care
services. Services currently include oral health education,
cleanings, fillings, extractions, and sealants. Dental services
are available to our patients and children ages 0 to 18 living in
Strafford County with Healthy Kids Gold or Silver.

This program continues to educate the public (including people at
risk) about how HIV and other sexually transmitted diseases are
spread, and how people can protect themselves from becoming
infected. To achieve this, a variety of services are used. They
include: seminars and health education programs, public
information campaigns that use the mass media to reach selected
audiences statewide, and operation of the two hot lines.

The Catastrophic Illness Program (CIP) is a state funded program
designed to provide financial assistance to individuals requiring
extensive medical treatment for specific medical conditions. The
Catastrophic Illness Program is part of the Office of Health
Planning and Medicaid. All inquiries to the program or
applications for assistance should be directed to the Program
Coordinator.
Individuals with Cancer, Hemophilia, End Stage Renal Disease,
Cystic Fibrosis, and Spinal Cord Injuries requiring extensive
medical treatment and who are in need of financial assistance may
apply to the program. Available assistance is limited to $2,500
per client per fiscal year, (July 1 through June 30).
Any individual or agency can request an application from the CIP
office by stating the name and condition of the individual in
need of assistance. Applications will be sent out and should be
completed and returned along with a recent 1040 income tax form
or other means of income verification. The applicant will be
notified, in writing, as to his/her acceptance or denial to the
program.

Serves children ages 0 - 16, and accepts Medicaid. Has a central
based clinic offering outpatient occupational speech and physical
therapy and pediatric therapy services. Also offers
speech/language pathology and occupational therapy. The program
offers experienced pediatric therapists, center-based therapy, a
pool for therapy, and home & community consultation. Programs
include sensory integration therapy, neuro developmental therapy,
and evaluations for therapeutic stimulation, feeding and
swallowing. Has a satellite office in Hampton, NH.

The Chase Home for Children is an intermediate level group home
licensed to serve up to 25 male and female adolescents who, for a
variety of reasons cannot live at home, and need a therapeutic
living environment more structured than a family setting.
Residents at the Chase Home are referred through the Division of
Children Youth and Families (DCYF) for abuse and neglect cases;
and the Division of Juvenile Justice Services (DJJS) for cases of
delinquent youth or CHINS - Child in Need of Services. All
placements are made through the court.
The Chase Home is a community based program. As such, all
residents attend public school and receive therapy and medical
services in the community. Residents also participate in
recreational and cultural activities and are active in community
volunteerism. The philosophy of the Chase Home is based on the
fundamental principle that children need a positive and
structured environment that fosters the development of strength
and character within the spirit of each individual.
Through a therapeutic environment we strive to help the resident
improve self-esteem and increase self-reliance. By building on
the individual's strengths, the Chase Home provides opportunities
to help the youth develop skills for positive relationships and

Apartments in Somersworth provides supportive services to any

Cross Roads House (located in Portsmouth) opened its doors in
1982 to assist homeless individuals and families. We have since
become the largest emergency and transitional shelter in New
Hampshire, serving eastern NH and southern Maine. We house up to
107 people on any given night, and our emergency shelter is
available 24 hours a day, 7 days a week. Cross Roads offers food,
shelter, and a proven program that helps our residents achieve
their goal of independent and sustainable housing. In fact, the
majority of people who have participated in our transitional
program leave Cross Roads House for permanent housing, equipped
with tangible goals and the skills to achieve them. Our mission
is to provide secure, transitional shelter for those trying to
break the cycle of homelessness. Listed below are just some of

2 fully equipped exercise rooms, containing a Universal
Conditioning machine, Olympic machines, treadmill, rowing
machine, and exercycles. The Indoor Pool facility is comprised of
a regulation swimming pool; a separate diving pool; sauna &
exercise equipment on deck. There are separate locker and shower
facilities at both the Gym & Pool.
Guppey Park is located on Portland Avenue and features an Ice

At Dover Rehabilitation & Living Center we offer something
unusual for a long-term care provider... short term care. Our
intensive rehabilitation programs provide a bridge to home,
easing the transition, or, as an alternative to acute
hospitalization. We provide a variety of services; from
post-surgical recovery to stroke recovery, from treatment for
orthopedic conditions to cancer, from cardiopulmonary care to IV
services. For patients of all ages, from short to longer-term
care, we can respond to many needs beyond traditional skilled
nursing services. Our staff helps patients quickly and
comfortably recover, minimizing inpatient stays. To that end, our
program addresses the physical, emotional, and social needs of
each patient. Our treatment team includes on-staff therapists,
specially trained nurses, a registered dietitian, activities and
social services staff. Together, they create a treatment plan
focused on patient-defined goals. Stays for our sub-acute medical

Adolescent Substance Abuse Group - Peg Correia,
M.Ed., LADC, and Sally Wool, M.Ed., LADC, are offering an
educational program for Adolescents on substance abuse and
recovery. This open-ended, on-going group, will meet on
Wednesdays from 3:30 to 5:00pm starting in October. Both
facilitators have extensive experience working with teens at all
stages of the continuum, from experimentation to addiction. Each
adolescent must attend one individual session with a parent or
guardian before joining the group. At this time, recommendations
will be made for the number of meetings the teen will be
encouraged to attend. The fee is $45 per group, with a number of
sliding fee spaces (based on individual need) available. Space is

Adult Survivors of Child Sexual Abuse - An ongoing
therapy group for women or men. A screening interview prior to
acceptance into the group is required.
Anger Management for Women - This is an ongoing
group for women wanting to learn new tools to manage their anger.
Depression Support Group - Open to anyone
suffering from depression. Prior screening interview is required.
DWI Aftercare - For individuals who have completed their local
Impaired Driver Intervention Program and are required to complete
aftercare prior to license reinstatement.
All groups are facilitated by trained therapists. All
groups are small in size with a maximum of six members. The fee

organization providing primary medical services, social services,

NE Emergency Response Systems (NEERS) is a locally based provider
of personal emergency response systems (commonly called
"help" buttons, Lifelines, or medi-alerts) for people
who are elderly, disabled, or in some way at risk. In case of an
emergency, a push of a button (worn as a pendant or on a
wristband) immediately connects the individual with our 24-hour
response centers. Within seconds, help is on the way.
NEERS also has a Fall Detection Device, a small sensor which is
worn on the waist and automatically calls for help if someone has
fallen.
NEERS' newest service, SafeChek, is a personal telephone call
from a friendly operator several times a day (individually
customized) to reassure or remind subscribers to take
medications, go to doctor's appointments, physical therapy, etc.,
or simply check in to make sure subscriber is all right. If there
is no answer after several calls, we will notify a relative or
other designated responder. No special equipment, no wires, no
help button and no speaker unit are required! All systems are
